Description

Summary

Forms4health™ is an intuitive, easily integrated, electronic smart forms platform, facilitating paperless working across health and social care. Forms4health is a responsive, versatile and interoperable platform that delivers digital transformation.

Full description

Forms4health allows an organisation to quickly and easily design their own smart eConsultations. The flexible design tool can be used to create forms for any purpose, supporting workflows across all areas of the organisation. The eConsultations themselves can include a range of sophisticated features, from branching logic (conditional questions), calculations, to clinical decision support. In addition to removing the need for paper forms, reducing costs, and saving time, the data gathered through forms4health can be easily analysed to provide valuable business intelligence. Using the in-built forms designer the organisation has complete control of how and when forms are created and implemented, without any reliance on external release cycles. The solution can be used as a stand-alone system, or be integrated with existing software such as GP Systems or Patient Administration Systems.

Implementation timescales

These are the typical processes and timescales to implement this Catalogue Solution:
As a cloud based solution forms4health eConsultation can be implemented within a day. The service can be launched within our secure Cloud environment and ready for customer use within 24 hours if using our library of eConsultations. The elements to implementation are usually around local form design, and testing and any local clinical safety approvals for the use of the form. Once a form has been designed and produced it it can be used across patient and/or professional settings. With intuitive forms design there is no training needed for the use of the forms. We deliver training for form design, for those organisations that want to develop their own eConsultations and sessions can be delivered remotely to speed implementation. The service/organisation would need to consider it’s own business processes for acting upon delivery of the eConsultation, but we can support these from our experience of deploying the product not only in primary care but in secondary care settings too.
